Polo Shirt Order Form - 2011


Members of OWPRA may order white polo shirts embroidered with OWPRA's logo. The shirts are Devon and Jones Peruvian Pima Pique Polo shirts, which are ordered from Queensboro. These are the same shirts that we ordered last year. If you are interested in ordering one or more shirts, please complete and submit this form as soon as possible.

Please note the following:

  • The shirts will be embroidered (not silk-screened) with OWPRA's logo. An image of the logo is below.
  • We must order a minimum of four shirts. If we do not receive orders for at least four shirts, no shirts will be ordered and any money received will be refunded.
  • You must submit payment in full (see below) before your order will be considered complete.
  • To guarantee shirt delivery before the beginning of the season, this form must be submitted and payment received before Friday, July 22, 2011.

  • Shirts are $30.00 each, regardless of size, and this includes any delivery costs.
